The Licensed Clinical Social Worker position is an integral part of the behavioral health care team and will work directly with the clinical care team and health coaches to meet our members' mental and behavioral health needs. This position will serve the PeopleOne Health offices in their designated area.SUPERVISORY RESPONSIBILITIES:This position provides supervision to others: NoESSENTIAL JOB FUNCTIONS:Support clinical care team with SBIRT implementation (Social Worker will be fully trained in SBIRT)Engage in care coordination for members to access our therapy and psychiatry partnerships to connect members with mental health servicesCollaborate with a multidisciplinary care team in service of membersProvide Social Determinants of Health screenings, resources, and referralsEngage in community outreach to build relationships and partner with local organizationsCoordinate with local substance use treatment programming for member assessments and referralsProvide education for members and employees on current standards of care in mental health
