This role is responsible for performing evidence-based telephonic symptom assessment, determining the appropriate level of care using approved clinical protocols, and ensuring patients receive safe, timely, and appropriate access to services. The Triage RN works collaboratively with providers, Care Navigators, Medical Assistants, and other members of the interdisciplinary care team to improve patient access, reduce unnecessary emergency department utilization, support continuity of care, and deliver an exceptional patient experience. This position is primarily non-patient-facing and requires strong critical thinking, communication, and clinical judgment.SUPERVISORY RESPONSIBILITIES:This position provides supervision to others: NoESSENTIAL JOB FUNCTIONS:Clinical TriagePerform telephone and electronic symptom assessment using approved clinical decision support tools and evidence-based triage protocols. Determine the appropriate disposition, including: Same-day appointment, Next-day appointment, Scheduled office visit, Emergency Department, and 911 activationRecognize time-sensitive and emergent conditions requiring immediate intervention.Escalate complex or uncertain cases to the appropriate provider.Assess reported symptoms and escalate patients requiring provider evaluation or emergency care.Care CoordinationCollaborate with Patient Care Coordinators, Care Navigators, and Medical Assistants to ensure patients are scheduled appropriately based on clinical urgency.Coordinate care transitions following emergency department visits or hospital discharge.Assist in closing care gaps by ensuring appropriate follow-up after abnormal findings or acute illness.Support continuity of care by communicating recommendations to providers and clinical staff.Clinical CommunicationProvide patient education regarding symptom management, medications, and home care.Reinforce provider treatment plans and discharge instructions.Communicate professionally with employers, pharmacies, specialists, and other healthcare partners as appropriate.Serve as a clinical resource for non-clinical staff regarding patient concerns.DocumentationDocument all patient interactions accurately and in a timely manner within the electronic health record (eCW).Ensure documentation supports continuity of care, quality reporting, and regulatory compliance.Maintain confidentiality and protect patient information in accordance with HIPAA.Quality & ComplianceFollow all organizational clinical policies, nursing standards, and evidence-based practice guidelines.Participate in quality improvement initiatives related to access, patient safety, and patient experience.Monitor for trends requiring escalation to clinical leadership.Maintain compliance with HIPAA, OSHA, CLIA (as applicable), organizational policies, and nursing standards.CollaborationCollaborate with Interdisciplinary Team – Communicate with primary care providers, registered dietitians, Medical Assistants, and care coordinators to ensure seamless handoff and follow-up.Participate in clinical huddles, case discussions, and workflow improvement initiatives.Support organizational goals related to value-based care and patient access.Champion Access and Experience – Serve as a welcoming, accessible point of contact for patients and contribute to a positive member experience.Hours of availability to work between 7 a.m. and 7 p.m. during the week.Participate in quality improvement initiatives.Other job duties as assigned
